CANCELLATION/LATE POLICY

We require 24-hours notice for cancellation of any

appointments. Missed appointments are billed at

100%.


Charges will be applied to your account.

This is NOT covered by OHIP, WSIB, or insurance.

Attending your therapy appointments is important

to your rehabilitation.

PAYMENT

Payment may be made by cash, Interac,

Mastercard, Visa or cheque.


Prices are subject to change without notice. Some patients may require pre-approval for third party billing. If claims are denied the patient is responsible for payment.


The patient is responsible for reviewing their insurance

funding limits and monitor accordingly.

CONSENT

All information gathered for treatment purposes is

confidential except as required or allowed by law or

except to facilitate diagnosis or treatment. You will

be asked to provide written authorization before the

release of any information.
